Home Health

As Medicare-Certified providers, our professional staff provide compassionate individualized care to people of all ages in the comfort of their homes. They work closely with family members or other caregivers on how to continue care between professional visits, creating a strong sense of empowerment.

Eligibility for Home Health Services

To meet the requirements of Medicare and other insurance sources, patients must be considered homebound to receive home health services. This means there should be a normal inability to leave home, and leaving home would be difficult and take considerable effort. The patient may be considered homebound if the absences from the home are infrequent or for periods of relatively short duration, or attributable to the need for health care treatment.

Hospice

Hospice care provides end-of-life care when the patient’s condition is considered terminal and the patient is no longer receiving curative treatment. A terminal diagnosis is defined as the patient having six months or less to live if the condition runs its normal course. The hospice team works with patients to manage pain and symptoms so that their remaining time may be spent in peace and dignity.

Dedicated to quality of life, hospice offers a support system of professional staff and volunteers who assist the patient and their family in meeting their physical, emotional, social and spiritual needs. Hospice care stresses the importance of human needs that go beyond the physical care of the patient. Hospice neither hastens nor postpones death; hospice affirms life and regards dying as a normal process.

For their optimal comfort, patients and families are supported in their own homes whenever possible. For those times when it is not possible for end-of-life care to be provided at home, Ministry Home Care has alternative homes specifically designed to meet the residential needs of individuals receiving hospice care.

For individuals who are unable to remain at home during their final months of life, Dr. Kate Hospice provides care to patients at our hospice home in Woodruff. The peaceful residence is tucked in a scenic wooded area on the edge of town.

The House of the Dove is a beautiful facility constructed to provide care for all levels of hospice patients - Residential, Acute and Respite. With 12 patient suites, the home is able to meet the broader needs of people in the Marshfield region.

Hospice is about living each day to the fullest. It's about helping individuals dying of terminal illness live each day in a pain-free environment, with dignity and peace. Home Infusion Therapy

Ministry/ Affinity Home Care Plus/ Wheaton Franciscan Infusion Pharmacy can coordinate comprehensive therapies, nursing services and supply delivery to meet your individual needs. Our team provides:

  • Nutritional support
  • Neuropathies/CIDP
  • Infectious disease treatment
  • Palliative care (pain management)
  • Oncology treatment
  • Transplant care
  • Blood disorder treatment
  • Pediatric infusion care

Available infusion therapies include:

  • Parenteral Nutrition (PN/TPN)
  • Antibiotic, Antiviral and Antifungal agents
  • Immunoglobulin therapy (IVIG/SQIg)
  • Inotropic medications for cardiac patients
  • Factor products
  • Alpha-1 Antitrypsin (AAT)
  • Remicade therapy

For medications that require professional monitoring, our convenient ambulatory infusion suite provides a comfortable, private setting to receive these therapies. The home infusion therapy team is comprised of pharmacists, nurses and dieticians. Support staff coordinate, administer and monitor home infusion therapies and services. Physicians work with our team, overseeing every aspect of a patient’s treatment.

GRIEF SUPPORT

Our hospice programs provide grief support services to anyone in the community, regardless of their involvement with hospice services prior to their loved one's death.

For surviving family members of hospice patients, we provide support for at least 13 months following the death - helping them through those painful firsts: the first Thanksgiving, first holidays, first birthday and first anniversary of the death.

In addition, support groups, educational seminars and special remembrance events are open to everyone in the community. Our compassionate bereavement coordinators and specially-trained volunteers share their resources and expertise with individuals, organizations and other professionals in the communities we serve to bring healing to all who need it.

Our grief support groups are open to all, and are provided free of charge.

Drop-in Support Groups

These grief support groups are monthly adult gatherings for individuals who have experienced the death of a loved one. These gatherings provide a caring community for learning about grief, finding support, sharing memories and identifying feelings. There is no fee to attend, no pre-registration is required and all adults are welcome.

Specialty Grief Groups

Death of a Partner (L.O.S.S)

This six-week series provides information and support to help participants cope with the multitude of feelings, adjustments and day-to-day concerns experienced after the death of a spouse, partner or companion.

For Men Only: Men Journeying through Grief

Affinity Visiting Nurses invites all men who have experienced the death of a loved one, whether recently or long ago, to join others in this ongoing opportunity to receive support for their grief. Although the experience of grief is certainly not limited to men alone, there are often differences in the way men express and deal with their grief, and the feelings that accompany it. Some of the messages men have received from our culture and how they are expected to act and respond to grief and loss can become stumbling blocks to their journey and healing through grief. The evenings consist of a simple supper, grief information, resources, videos and discussions led by specially-trained male professionals.
 
Meetings are scheduled on the first Wednesday of every month, 6 - 8 p.m. at the Affinity Visiting Nurses office, 816 W. Winneconne Avenue, Neenah (In the Fox Point Strip Mall). There is no fee to attend. Pre-registration is appreciated for meal planning purposes, but not required.

A Time to Mourn

This is a small group designed for adults who have lost a loved one and are finding it difficult to work through their grief. This six-week session will provide an atmosphere of understanding and support in addition to providing information on the grief process. These sessions are free of charge and are co-led by professionals trained in grief support.

Surviving Spouse Social Outings

In addition to the regular gatherings of the social group, various activities for small groups are offered. Included in these outings are potlucks, bowling, golfing, playing cards and attending the theater.

Supportive Souls

This grief group is only for parents who have lost a child at any age. The death doesn't have to be recent.
